SiteBuilder provides various templates for entering pages with contact forms. These can be chosen when entering a new page or at any other time, by changing type of page, by clicking the “Edit” button in “Website Manager”.
You can choose to enter contact Forms, Forms for requesting information, Complaint Forms etc..

Once you have chosen the type of form that you wish to use, you must configure the page that will then host it.

Simple Form

You need to:
Indicate the title or name of the page where the form will be placed;
Enter the text that you wish to display to the users that open this page.

Once you have typed the title and the message that you wish to display above the form, you need to indicate the name of the fields of the form that the users need to fill in;

You can then choose a message to place under the form and the address (or the addresses) where you would like to receive the details entered in the form:

N.B.: The details entered in the form will be sent by email to the specified address (or  specified addresses). As the sender of such messages, the system will use the email address that appears in the header or footer of the website created through SiteBuilder. To change the sender address of such messages you will therefore need to change the template by providing a different email address.

Once completed the above the system will allow you to create a message that will be displayed when the users have finished filling in the form: here you can also choose to enter a simple text  directly in the field found in the page, or enter a more complex text with links and images, by selecting “Advanced Text, Link & Image Insertion”.

Last of all click on submit to confirm the creation of the page with the form.

A preview of the page will then be displayed with the form that you have just created: to  confirm it, click on “Approve”.

Advanced Form

Besides the simple form indicated above, the system allows you to create pages with forms featuring more information and options.
To enter an advanced form in a website page you need to:

Indicate the title or name of the page with the form: the name that you choose will be the same name that is displayed in the menu found in the home page;
Enter the text that you wish to display to the users that open this page. In this case you can choose to enter a simple text, or a more complex text, including images and links, by clicking the provided option, with which you can open a dialogue box which allows you to enter html text, images and links to place above the contact form.

Once you have indicated the title and the message that you wish to display above the form, you need to indicate the fields of the form that the users need to fill in and the settings for filling in such form. The advanced form in fact, besides specify the names of the fields included in the form, allows you to set ways in which it must be filled in, choose the type of field that you wish to enter like text field, dropdown menu, check box and radio button.

At the top of the page, you will find a useful guide which shows an example of the various types of fields that you can add to the form. Once you have chosen the types of fields you wish to use and the name, you can then choose the number of options that you wish to display to the user, mainly for dropdown menus, radio buttons or check boxes.
